Careplans are used by GPs for patients with ongoing chronic conditions such as chronic diseases, complex needs and terminal illnesses.
During your careplan appointment you will first see the nurse for general observations and assessments for allied health referrals.
Afterwards you see your doctor and discuss your needs further and get referrals signed off as well as any prescriptions related to your chronic condition/s.
- Careplan appointments are Bulk Billed under Medicare
- Duration 1 hour (Please allow for this during your appointment)
Who is eligible?
Those who have at least one or more chronic conditions e.g. arthritis, anxiety, asthma, diabetes, cancer, mental illness, heart disease, kidney disease etc.
GPMP= Genral Practioner Management Plan
TCA= Team Care Arrangement
TCA referrals can be sent to related allied health professionals by your GP for management of chronic conditions (e.g. physio, chiro, therapists, podiatrists, dietician etc.)
You will be allocated 5 subsidized visits to your choice of related conditions referral example: 2 physio visits & 3 Chiropractic visits.
Regular review of your careplan allows for you and your medical team to access whether your goals of treatment are being met. generally, a review happens every 3-6 months.
If you are unable to make it to the practice, appointments may be available via Telehealth consultation.
